Claims Process

In the event of an incident leading to an aviation hull insurance claim, the following documents will be required to process the claim:

  • Claim notification to the Insurance Broker/ Company
  • Duly completed claim form.
  • Original cover note.
  • Original police abstract report.
  • Pilot’s license
  • Engineers repair estimate
  • Original ownership documents.
  • Copy of Certificate of incorporation for companies and ID’s for individuals.
  • Duly executed ownership transfer forms (*2) in case of total loss claim

In the event of an incident leading to a marine hull insurance claim, the following documents will be required to process the claim:

  • Claim notification to the Insurance Broker/ Company
  • Duly completed claim form.
  • Original cover note.
  • Original police abstract report.
  • Pilot’s license

Claims Process

In the event of an incident that may lead to a marine insurance claim, the following documents will be required to process the claim:

  • A duly completed Claim form.
  • Original certificate/cover note.
  • Original Bill of lading/Airway Bill.
  • Supplier’s invoice, Packing List, Port examination voucher
  • Short-landing certificate (in case of loss discovered at discharge)
  • Import declaration form and Port Release order
  • Defective package Receipt (In case of damaged goods)
  • Inland Carriers delivery receipt Notes at final destination.
  • Copies of correspondence from the insured/their agents regarding liability exchanged with; Ships Agents or Inland carriers or Kenya Ports Authority or Kenya Airports Authority.
  • Evidence of the value of goods carried e.g. Invoice/ purchase receipt.
  • Computation of value of damaged goods/ repair or replacement estimate
Claims Process

In the event of an incident that may lead to a goods in transit insurance claim, the following documents will be required to process the claim:

  • A Duly completed Claim form.
  • Police abstract report (in case of theft, malicious damage or accidental claims)
  • Delivery note, packing list/ goods manifest
  • Evidence of the value of goods carried e.g. Invoice/ purchase receipt.
  • Computation of value of damaged goods/ repair or replacement estimate
  • Statement on the circumstances of the loss from the driver and loader.
  • Witness statement.
Claims Process

In the event of non-compliance such as failure to pay duty for goods released for sale in the local market, the guaranteed authority recalls the bond and issues a demand note to the insurance company reimbursement of the duties payable or cost of deportation as the case may be.

  • Original duly executed bond
  • Quotations for repair or Replacement of damaged property.
  • Statement on the circumstance of the loss.
  • Witness(es) statement(s).
Claims Process

In the event of an incident that may lead to a terrorism & political violence insurance claim, such claims would ideally be adjusted by a loss adjuster to determine the amount of loss and the undernoted documents would be required:

  • A Duly completed Claim form.
  • Police abstract report.
  • Quotations for repair or Replacement of damaged property.
  • Statement on the circumstance of the loss.
  • Witness(es) statement(s).

In the event of an incident that may lead to a plate glass insurance claim, the following documents will be required to process the claim:

  • A Duly completed Claim form.
  • Police abstract report (in case of theft or malicious damage claims).
  • Replacement estimate/ invoice/ proforma invoice.
  • Quotation for damaged works/ repair or replacement estimate
  • Statement on the circumstances of the loss from the insured.

 

WIBA Claims Process
  1. Notification - By the insured to the District Occupational safety and Health Office by submitting DOSH 1 with a copy to Verge Insurance Brokers
  2. Claim Documents:
  • Claim notification to the underwriter
  • Duly completed accident report form
  • Original medical Bills
  • Second medical opinion if required by the Insurance Company
  • Certified copy of Death Certificate if fatal injury
  • Identity Card in the event of fatal injury
  • Police Abstract Report in case of accident or murder

Letter of administration

Claims Process

In the event of an incident that may lead to a Personal accident claim, the following documents will be required to process the claim:

  • Claim notification to the Insurance Broker/ Company
  • Duly completed personal accident claim form and medical certificate duly executed by the treating doctor.
  • Copy of students ID
  • In case the accident is fatal: a Death Certificate, a copy of National Identity Card and Police Abstract Report.

In the event of an incident that may lead to a Personal accident claim, the following documents will be required to process the claim:

  • Claim notification to the Insurance Broker/ Company
  • Duly completed personal accident claim form and medical certificate duly executed by the treating doctor.
  • Copy of payslip for the month just before the accident (for employees only)
  • In case the accident is fatal: a Death Certificate, a copy of National Identity Card and Police Abstract Report.
  • Original Medical Bills.
Claims Process

In the event of an incident that may lead to a Personal accident claim, the following documents will be required to process the claim:

  • Claim notification to the Insurance Broker/ Company
  • Duly completed personal accident claim form and medical certificate duly executed by the treating doctor.
  • Original Medical Bills and discharge summary.
  • A medical certificate duly executed by the treating doctor (In case of disability claims)
  • In case the accident is fatal: Death Certificate, copy of National Identity Card and Police Abstract Report.
  • A post-mortem report will be required in case of murder or sudden unexplained death
Carrier’s liability claims Procedure

Documents required for carriers liability claims

From the customer/client
  • A Duly completed Claim form.
  • Police abstract report.
  • Delivery notes, packing list, invoice/ purchase receipt.
  • Statement on the circumstance of the loss by the driver/loader.
  • Witness statement.
  • Any demand letter of summons received from the third party lawyers. Please do not respond to any demand letter or summons, forward them to Verge Insurance Brokers on the same day.

  • Acceptance of loss form/ Discharge voucher
  • Bank Transfer Confirmation slip i.e. RTGS or SWIFT

  • Claim notification to the Insurance Broker/ Company to facilitate the appointment of a loss adjustor
  • Duly completed Claim form and particulars of the claim and necessary evidence
  • Police abstract report
What will the Insurance Company provide?
  • Court judgement or out of court agreement
  • Computations of the compensation payable.
  • Bank Transfer Confirmation slip i.e. RTGS or SWIFT directly to the third party advocates

  • Claim notification to the Insurance Broker/Company to facilitate the appointment of a loss adjustor.

What will the Insurance Company provide?
  1. Details of appointed defense lawyers.
  2. Court judgement or out of court agreement
  3. Computation of the compensation payable.
  4. Bank Transfer Confirmation slip i.e. RTGS or SWIFT directly to the third party advocates
  5. Subrogation rights form for client’s execution to facilitate recovery.
